1 How to apply for the learner permit

第一类：如何申请实习驾照

1 For a first time learner permit application you must supply:

Application form for learner permit D201 (Fully completed )

Evidence of PPSN  

税卡证明

Driver Theory Test pass certificate (dated within 2 years)

NDLS Eyesight Report form D502 (Fully completed and dated within 1 month)

2 To renew a learner permit you must provide

 延续实习驾照的人员需提供：

Application form for learner permit D201 (Fully completed)

Current/ most recent Learner permit 

当前或近期的实习驾照

Evidence of PPSN if required. 

如果需要请提交税卡（PPSN）证明

If you do not have your most recent learner permit you must present a completed lost licence declaration (question 17 on D201) together with valid photographic ID

如果没有最近的实习驾照的话你就必须提交一份完整的《许可遗失声明》（17项表D201），附上一张有效的身份照片

If applying for a third or subsequent Learner permit, documentary evidence must be submitted that you have either sat a driving test (statement of driving test outcome dated within two years) or an acknowledgement of a forthcoming test.

如果申请到第三次或多于三次的实习驾照，必须提交书面证据证明你通过了理论测试（两年内的驾驶理论测试结果日期）或即将进行的测试。

NDLS medical form (dated within 1 month) if required

3 To add a category to a learner permit you must provide

If you do not have your most recent licence you must present a completed lost licence declaration (question 17 on D401) together with valid Photographic ID

Evidence of PPSN
